FIT at 45 Program

This program aims to help the underserved populations across Northern Arizona to reduce gaps in patient access to colorectal cancer screening and associated outcomes. Preventative cancer screening tests, education, and resources are available to patients. Diagnostic testing such as colonoscopies, biopsies and other tests are also available if screening results are abnormal. Referrals are made for treatment as necessary.

Since education plays a vital role in avoiding late-stage diagnosis, patients are encouraged to be advocates for their own health whenever possible. The FIT at 45 Program at North Country HealthCare (NCHC) is funded through the Health Resources and Services Administration with a grant from the Health Center Program’s Accelerating Cancer Screening initiative.

Who is eligible for free colorectal cancer screening or navigation?

  • Active and established patients of North Country HealthCare at any of our clinics in Apache, Coconino, Gila, Mohave, and Navajo Counties.
  • New patients of North Country HealthCare who have established care.
  • Men and women between 45–75 years of age.
  • Low-income, uninsured, underinsured, and insured patients with significant barriers to obtaining colorectal cancer screenings.
