School-Based Health Services (Flagstaff)

Sometimes it can be difficult for patients to get to their medical appointments at North Country HealthCare, especially those patients who are students. It is our goal to ensure no person is without access to primary care. Sometimes that means taking the care to our patients, wherever they are!

North Country HealthCare has partnered with the Flagstaff Unified School District to offer primary care appointments to a school nearest you. We treat patients in our state-of-the-art mobile medical unit.

  • Management of Acute and Chronic Conditions (asthma, diabetes, etc.)
  • Urgent care for sudden illness or injury
  • Sports Physicals

Once completed please submit forms to your schools front office or email a completed form to schoolforms@nchcaz.org.

    • Tuesday Afternoons
  • Thomas Elementary School (3330 E Lockett Rd, Flagstaff, AZ 86004)
    • Every Other Tuesday Morning
  • Killip Elementary School (2300 E 6th Ave, Flagstaff, AZ 86004)
    • Every Other Tuesday Morning

Payment

Most insurance plans are accepted, including AHCCCS (Arizona’s Medicaid program) and Medicare. We also have a sliding fee scale for those who qualify.

North Country HealthCare has certified application counselors to assist with enrolling in AHCCCS health insurance, Supplemental Nutrition Assistance Program (SNAP) and Temporary Assistance for Needy Families (TANF cash assistance).
