School-Based Health Services (Flagstaff)

Sometimes it can be difficult for patients to get to their medical appointments at North Country HealthCare, especially those patients who are students. It is our goal to ensure no person is without access to primary care. Sometimes that means taking the care to our patients, wherever they are!

North Country HealthCare has partnered with the Flagstaff Unified School District to offer primary care appointments to a school nearest you. We treat patients in our state-of-the-art mobile medical unit.

  • Vaccine consent forms must be filled out and signed by parents for all students under 18.
  • Students under age 12 must have a parent, guardian or approved emergency contact with them at vaccination with a signed parental consent form.
  • On-site registration will be accommodated as schedule allows.

Payment

Most insurance plans are accepted, including AHCCCS (Arizona’s Medicaid program) and Medicare. We also have a sliding fee scale for those who qualify.

North Country HealthCare has certified application counselors to assist with enrolling in AHCCCS health insurance, Supplemental Nutrition Assistance Program (SNAP) and Temporary Assistance for Needy Families (TANF cash assistance).
