Care Management

Care Management is one of the many no-cost services you’ll receive as a patient at North Country HealthCare. As your medical home, we are committed to helping you receive the care and support you need to manage your condition and live life to the fullest! We know that living with a chronic condition is not easy; it can be difficult and confusing to sort through the information and treatment options.

Care Managers are people who understand the needs of people living with chronic conditions such as diabetes, asthma, high blood pressure and heart conditions. Care Managers can provide the extra help to follow through with your provider’s instructions. They provide personal attention in helping you meet your health goals.

Care Managers bridge the gap between you and your many providers. They work with you to coordinate the healthcare you need to manage your condition.

Care Managers can help you:

  • Understand your diagnosis and things you can do to manage it
  • Support you in making and showing up to medical appointments
  • Follow through on your medication or other prescription instructions
  • Link you to community resources, such as support groups
  • Discuss possible transportation choices to help you get to appointments
  • Develop an individualized plan to help you meet your health goals
