North Country HealthCare‘s family practice providers are dedicated to treating your overall health. They focus on building continuous patient-provider relationships. This speciality is not limited to a particular organ or disease. Family practice providers care for patients of all ages and deliver a range of acute, chronic and preventive medical care.

North Country HealthCare is a patient centered medical home (PCMH). This means we practice a team based approach to healthcare. Our goal is to provide comprehensive and coordinated medical care to maximize health outcomes for you and your family. We also have health coaches and care managers on our team that may contact you about your healthcare needs.
