We just launched a new Program Eligibility Tool – check it out!
This new tool will allow people to see what local assistance programs they may qualify for.
After completing a free screening, an eligibility specialist at North Country HealthCare can help you apply for assistance programs.
North Country HealthCare just launched its new program eligibility tool, which allows anyone to easily pre-screen their eligibility for assistance programs offered by North Country HealthCare and various state and federal programs.
This free tool can be accessed using the “Program Eligibility” link on the homepage at northcountryhealthcare.org. Once there, users will enter basic information for themselves, a child, or another person. After answering a short series of questions, a results page will appear, displaying which community assistance programs they may qualify for, based on the information they provided.
Users can follow the links provided to learn more about each specific program. They can also request a free appointment with an eligibility specialist at their local North Country HealthCare. Eligibility specialists can assist with applying for certain programs, like Medicaid, Supplemental Nutrition Assistance Program (SNAP), or North Country HealthCare’s Sliding Fee Scale.
Eligibility specialists can also help find more information about applying for other programs that person may qualify for.
