North Country HealthCare is a community health center. Community health centers keep communities healthy. We would love to be your partner in health. Becoming a patient is easy – you can request an appointment right now by visiting us online at northcountryhealthcare.org.
All of North Country HealthCare’s locations are always accepting new patients! We have a variety of highly-skilled providers for patients to choose from.
If you are not a already a patient of North Country, you can request an appointment online or call the location nearest you. Before calling, you’ll want to have your insurance card on-hand.

We accept most commercial, Medicare and Medicaid insurance plans. Below is a list of some of the major insurance plans North Country HealthCare is contracted with. Because healthcare coverage and insurance plans are constantly changing, it is important to check with your insurance provider before scheduling an appointment to confirm your insurance will pay for services at North Country HealthCare.
*We also accept AHCCCS plans for the following counties: Apache, Coconino, Gila, Mohave, Navajo and Yavapai
North Country HealthCare is committed to providing access to the very best medical and dental care for all ages, regardless of insurance or financial status. If you are uninsured, our Health Benefits Advocates will work with you to see if you are eligible for any available insurance coverage and assist you with enrollment – at no cost to you.
We also offer a sliding-fee scale that allows our uninsured patients to pay variable rates (eligibility is based on income and family size). We also offer discounted fees and programs that can assist when insurance doesn’t cover a service.
