A “Welcome to Medicare” visit is a one-time preventive physical exam covered by Medicare within the first 12 months of having Medicare Part B. It’s a chance for your doctor to review your health, discuss preventive services, and create a plan for future care. This visit is a good opportunity to learn about your health and how to stay healthy as you transition into Medicare. If you are in the first 12 months of having Medicare for health insurance, call us today to make this one time, no cost, “Welcome to Medicare” visit.
After the first 12 months on Medicare, everyone with Medicare or a Medicare Advantage Plan is eligible, every year, for an Annual Wellness Visit. This visit takes about 40 minutes and is free of charge.
The Annual Wellness Visit is an in-depth chart review making sure your clinician has the correct information on your health history, including your medications and diagnoses. The visit will also address topics like nutrition, falls, and preventive care. By addressing these issues during a wellness visit, you and your clinician can focus on your current health status and any new questions or concerns at your regular office visits.
The Annual Wellness Visit does not include a physical exam.
Yes! If you have seen one of our primary care providers in the past 12 months, and you have a smart phone or computer, you can have this visit from home.

Yes! Call your regular clinic and ask for a Medicare Annual Wellness Visit appointment.
If you have seen one of our primary care providers in the past 12 months, your regular clinic may set up a in-clinic telehealth visits with one of our providers.
If you have not been seen in primary care in the past 12 months, your regular clinic can make an appointment for you with one of the clinic providers.
