A parent or guardian must be present for appointments at Killip Elementary School.
Please complete a consent form for your child to receive care in receive healthcare services provided by North Country HealthCare’s Mobile Unit. Students may submit the form to their school’s front office or email a completed copy to: schoolforms@nchcaz.org
Most insurance plans are accepted, including AHCCCS (Arizona’s Medicaid program) and Medicare. We also have a sliding fee scale for those who qualify.
North Country HealthCare has certified application counselors to assist with enrolling in AHCCCS health insurance, Supplemental Nutrition Assistance Program (SNAP) and Temporary Assistance for Needy Families (TANF cash assistance).
