School Based Health Services

North Country HealthCare offers medical services at schools in Flagstaff Unified School District through our Mobile Medical Unit (MMU) van to serve as an exam room.

  • Unexpected illness or injury
  • Management of acute and chronic conditions (asthma, diabetes, etc.)
  • Sick visits
  • Testing: Flu, COVID & RSV
  • Sports Physicals and more

Summit High School
4000 N Cummings, Flagstaff, AZ 86004

9:00 AM – 1:00 PM every Tuesday, except FUSD closures

Killip Elementary School
2300 E 6th Ave, Flagstaff, AZ 86004

2:30 PM – 5:00 PM every Tuesday, except FUSD closures

 

 

 

A parent or guardian must be present for appointments at Killip Elementary School.

Consent Forms

Please complete a consent form for your child to receive care in receive healthcare services provided by North Country HealthCare’s Mobile Unit. Students may submit the form to their school’s front office or email a completed copy to: schoolforms@nchcaz.org

Payment

Most insurance plans are accepted, including AHCCCS (Arizona’s Medicaid program) and Medicare. We also have a sliding fee scale for those who qualify.

North Country HealthCare has certified application counselors to assist with enrolling in AHCCCS health insurance, Supplemental Nutrition Assistance Program (SNAP) and Temporary Assistance for Needy Families (TANF cash assistance).
