• Determine pregnancy and refer you to a physician for prenatal care
• Answer prenatal questions and concerns
• Prepare for labor, delivery, infant and child care
• Prevent low-birth weight
• Get care for high-risk pregnancy
• Get access to community resources including AHCCCS, WIC and other family services
• Receive health education on topics ranging from prenatal care, parenting, breastfeeding, well-child checkups, safety, childcare and more

• Receive regular check-ups
• Get appropriate immunizations and establish a medical home
• Acquire early screens to identify developmental delays and receive referrals for treatment

Each family enrolled in Health Start is paired with a Community Health Worker who provides confidential personalized support, education, and advocacy in the comfort of the the family’s home. Community Health Workers are not just visitors; they’re integral members of the communities they serve. Your Health Worker will stand by you and your family throughout your enrollment into Health Start, and provide assistance and guidance as long as you stay enrolled or until your baby reaches two years of age.

Community Health Workers are specially trained in:

• Prenatal growth and development
• Breastfeeding
• Labor and delivery
• Premature warning signs and infant care
• Nutrition during pregnancy
• Parenting
• Child growth and development
• Benefits of preventive healthcare (immunizations and well-child checks)
• Car seats
• Safety in the home

Community Health Worker Visits:

• At least once a month during pregnancy
• Once before your baby is 2 weeks old
• When your baby is 2, 4, 6, 8, 12, 18 and 24 months old
• Additional visits as needed or requested
