Health Partners

Good health depends on more than just seeing a doctor. Our health is also strongly impacted by where we live and work. Health Partners is a clinic-based program that works one-on-one with patients and their providers to help move people towards greater well-being. Note: This program is only offered at our Flagstaff 4th St location.

Our team is here to ensure patients have the information they need to access community resources. Health Partners connects patients to a variety of social services such as:

  • Housing assistance
  • Job training
  • Hot meals and food boxes
  • Vision care
  • Childcare
  • Assistance with reducing medication costs
  • Affordable heating in the winter
  • Clothing
  • Low cost and free cell phones
  • Legal assistance

Desk hours: See clinic hours for Flagstaff – 4th Street.

Assistance is only offered in-person for the first visit. Follow-up visits may take place via phone.
