Schedule Your Baseline Blood Test

Appointments are now available for a free baseline blood test during our Small Business & Community Health Fair on Friday, April 5th, 2024. Blood tests are offered on a first-come, first-serve basis, so book yours today!

• First, select your appointment time on the calendar, then enter your information.
• Click the ‘Submit’ button only once and wait for the message “Your appointment has been successfully booked!” to appear.
• A confirmation email with appointment details will be sent to the email address provided. If you do not receive an email, check your SPAM folder.
• If a time slot is greyed out, there are no appointment available at that time.
