Health Partners (Flagstaff – 4th Street)

Good health depends on more than just seeing a doctor.  Our health is also strongly impacted by where we live and work.  Health Partners is a clinic-based program that works one-on-one with patients and their providers to help move people towards greater well-being.

Our team is here to ensure patients have the information they need to access community resources. Health Partners connects patients to a variety of social services such as:

  • Housing assistance
  • Job training
  • Hot meals and food boxes
  • Vision care
  • Childcare
  • Assistance with reducing medication costs
  • Affordable heating in the winter
  • Clothing
  • Low cost and free cell phones
  • Legal assistance

Desk hours: 9 am – 5 pm, Monday through Friday
Assistance is only offered in-person for the first visit. Follow-up visits may take place via phone.
