| FFHP/HIV Program Specialist | Kingman | We are seeking a Program Specialist in Kingman for the HIV/AIDS and the Fit at Fifty HealthCheck Programs. This is a full time position split between the HIV/AIDS and the Fit at Fifty HealthCheck Programs under the Outreach division of North Country HealthCare. The Program Specialist will conduct screening and implement the outreach activities as laid out in the contractual guidelines and work plan. St. Johns is located at the doorstep of the White Mountains in eastern Arizona. Work either 100% outpatient or choose to participate in the hospital call group for White Mountain Regional (http://www.wmrmc.com/) in Round Valley, AZ. If not participating in the hospital call group phone call is 1 in 12 weeks. No OB. Providers work Monday through Friday (4 days) 4 X 10 hour shifts. Part time is possible as well. During a 10 hour shift providers are expected to see 22-25 patients. NCHC received a federal grant and will be building a new clinic which should be completed by summer 2011. Providers may live in Holbrook or Winslow. | Nurse Practitioner - Ash Fork/Seligman | Ash Fork and Seligman (2 clinics 1 position) are separated by about 25 miles via I-40 located in Northern Arizona. Both towns are small rural communities. The ideal candidate will work 5x8 hour shifts (3 in Ash Fork and 2 in Seligman) or 4X10 hour shifts (2 in each town). During a 10 hour shift providers are expected to see 22-25, 8 hour shift 18-22 patients. Candidate may live in Flagstaff, Williams, Chino Valley, Kingman, Ash Fork or Seligman. Clinic is actively seeking a mission driven NP who has the desire to make a difference in the lives of the underserved. North Country HealthCare is the only medical clinic for a large catchment area. Ideal candidate will be comfortable working as a solo provider most of the time and have at least 1 year of comparable FNP outpatient experience. Office and support staff travels between the clinics and therefore are the same each day. Support staff is very experienced to include an RN. Patient mix is roughly Medicaid (55%), Medicare (15%), commercial insurance (15%) and uninsured patients (15%). 100% outpatient, no OB, phone call 1 in 20 weeks, FTCA medical malpractice provided. All North Country HealthCare clinics are loan repayment qualified! Clinic is staffed with 1 FP MD (leaving in October) and 1 part time PA.
