Job Description – Nurse Practitioner
Job Summary: A licensed and credentialed NP provides diagnosis, treatment, consultation, and follow-up for North Country HealthCare. The NP provides age and specialty appropriate medical care at the level of training achieved.
Job Duties:
Ash Fork and Seligman (2 clinics 1 position) are separated by about 25 miles via I-40 located in Northern Arizona. Both towns are small rural communities. The ideal candidate will work 5x8 hour shifts (3 in Ash Fork and 2 in Seligman) or 4X10 hour shifts (2 in each town). During a 10 hour shift providers are expected to see 22-25, 8 hour shift 18-22 patients. Candidate may live in Flagstaff, Williams, Chino Valley, Kingman, Ash Fork or Seligman.
Clinic is actively seeking a mission driven NP who has the desire to make a difference in the lives of the underserved. North Country HealthCare is the only medical clinic for a large catchment area. Ideal candidate will be comfortable working as a solo provider most of the time and have at least 1 year of comparable FNP outpatient experience. Office and support staff travels between the clinics and therefore are the same each day. Support staff is very experienced to include an RN. Patient mix is roughly Medicaid (55%), Medicare (15%), commercial insurance (15%) and uninsured patients (15%).
100% outpatient, no OB, phone call 1 in 20 weeks, FTCA medical malpractice provided.

Clinic is staffed with 1 FP MD (leaving in October) and 1 part time PA.

- Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.
- Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.
- Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues.
- Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications.
- Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with NCQA (National Committee for Quality Assurance), JCAHO (Joint Commission on Accreditation of Healthcare Organizations), and state regulatory standards.
- Facilitates evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receives and implements constructive directives.
- Analyzes new knowledge gained from conferences, workshops, professional literature, or "hands-on training" and assimilates this knowledge into clinical practice.
