At North Country HealthCare we believe that it isn’t what you do, so much as how you do it.

When a person understands his or her illness, that person has power over it.

Patient education and empowerment are essential pieces of our mission statement. Our providers believe that well-informed patients are healthier patients. People enrolled in our diabetes program have significantly lower average sugars than their peers. Similar benefits happen with other chronic diseases such as elevated blood pressure and high cholesterol. 

North Country has several behavioral health counselors on our medical team. They work with individuals and families to not only educate, but also to change lifestyles. Preventing a disease like diabetes is as important as helping someone learning to live with it. And if medication is needed, it isn’t enough for a person to be handed a prescription. We believe that a person who understands what a medicine does is more likely to live healthier.

Our behavioral health team works with individuals as well as families and small groups. Their range of services includes everything from tobacco cessation coaching to free community diabetes education classes. They also provide short term counseling services for stress, anxiety, depression and many other conditions.

They will work with you to:
     * Understand the relationship between physical health and wellbeing
     * Make lifestyle changes to improve health and wellbeing
     * Address stress, anxiety, depression and relationship issues
     * Create a plan to achieve better health outcomes
     * Manage health challenges and diagnoses
     * Provide referrals to outside agencies
     * Navigate specific health challenges in group sessions
